Minor piercing documents
For minor piercings, Elevated’s public policy says all piercings for minors must meet state health department identification requirements. Please bring everything listed below to the appointment.
- Parent or guardian present in the room at the time of the piercing.
- Parent or guardian valid photo ID.
- Minor child identification, such as state ID, military ID, school ID, yearbook, or another photo/name ID.
- The minor child’s birth certificate in hand. Pictures cannot be accepted; copies must be certified copies from the county clerk.
- If the adult is a legal guardian, bring legal guardianship paperwork along with the birth certificate and guardian ID.
The paperwork is used to match the adult’s name and legal authority to consent. If the required documents are missing or do not match, the appointment may not be able to proceed.
Children’s ear piercings
Children’s ear piercing appointments are scheduled with extra time so the piercer can move at the child’s pace and keep the experience calm, positive, and empowering for the family.
